Investigation found that the captain and first officer had worked beyond legal flight hour limits. During an attempted go-around after an aborted landing, the first officer tried to transfer control to the captain, but neither pilot properly executed the go-around procedure. Deficient training and safety programs at One-Two-Go Airlines contributed to the crew's failure.
On 16 September 2007, One-Two-GO Airlines Flight 269, operating a scheduled Bangkok–Phuket service, crashed into an embankment beside runway 27 at Phuket International Airport at approximately 15:41 ICT. The MD-82 burst into flames on impact, killing 90 of the 130 people on board, including one who died of burn injuries days later.
The crash prompted investigations by both Thailand's AAIC and the U.S. NTSB. Between 2009 and 2010, One-Two-Go Airlines was banned from operating in European Union nations due to safety concerns. The airline was ultimately fully rebranded as its parent company, Orient Thai Airlines, in July 2010.
