His professional career included playing in several leagues across the Middle East, such as the Iraqi Premier League, UAE Arabian Gulf League, Qatar Stars League, and Saudi Professional League, where he found significant success and gained recognition.
Mahmoud's international career took off in the early 2000s, with his first official goal being a crucial 89th-minute equalizer in the 2002 WAFF Championship final, which helped Iraq win in extra time. He continued to shine in regional competitions, scoring in the 2005 West Asian Games final to lead his team to another title. He became the national team captain in 2006, marking the start of the most successful period of his career.
The peak of Mahmoud's achievements was at the 2007 AFC Asian Cup, where he led Iraq to their first-ever win in the tournament. He played exceptionally throughout and scored the winning goal in the final, also earning the Golden Boot and Most Valuable Player awards. This historic win raised his profile beyond the region, earning him a nomination for the 2007 Ballon d'Or, where he finished 29th and became the only Iraqi player to receive such recognition.
In his later career, Mahmoud played in the 2009 FIFA Confederations Cup, participating in all three of Iraq's matches, and continued to score in regional tournaments, including the 2013 Arabian Gulf Cup final. He is the only player to score in four consecutive AFC Asian Cup tournaments (2004, 2007, 2011, and 2015). When he retired in 2016, Mahmoud was Iraq's most-capped player and their third-highest goalscorer. He later moved into football administration, becoming the second vice-president of the Iraq Football Association.
